Type of acceptable Birth Certificate:
“Long-form” Birth Certificate (aka Unabridged Birth Certificate)

Department of Home Affairs
Private Bag X114

Special things to note:
Applicants should also provide their full name given at birth, their date and place of birth, and both parents’ names, including the mother’s maiden name.
The request should specify that an English language copy is desired.
An Unabridged Birth Certificate would provide both parent’s names. An Abridged version is not acceptable since it is “short” or “fails” to state parental information. This is why the US Government requires the unabridged version.
